Hayden Panettiere’s cause of death has finally been made public, more than a month after the actress was found unresponsive in South Carolina. The 36-year-old star, known for Heroes, Nashville and the Scream franchise, died from the toxic effects of several substances, according to the Greenville County Coroner’s Office. Her death was officially ruled an accident.
The actress died on August 16 in Greenville, where she had been temporarily staying. Emergency responders were called at around 1:51 p.m. after a report of cardiac arrest. When they arrived, Panettiere was unconscious and in cardiac arrest. Despite advanced efforts to revive her, she was pronounced dead at 2:32 p.m.
The coroner determined that the cause was the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. The autopsy found no signs of trauma that contributed to her death, and officials classified the manner of death as accidental.
Police have also stressed that there were no initial indications of foul play or suspicious circumstances. At the same time, the Greenville Police Department said its investigation remains active and detectives are continuing to follow leads and gather information. The Drug Enforcement Administration has also been involved in the investigation.
Panettiere’s longtime on-again, off-again boyfriend Brian Hickerson and his brother Zach were present when she was found. According to the police report, Brian provided investigators with a bag containing medication that Hayden had reportedly been taking. Narcan was also administered after she was discovered unconscious, but emergency responders were unable to save her.

Hayden had spoken publicly for years about her struggles with alcohol and drugs. She began working as a child actress and eventually became internationally known after starring as Claire Bennet in Heroes. She later played Juliette Barnes in Nashville and appeared in Scream 4 and Scream VI.
Her career also included films such as Remember the Titans, Ice Princess and Bring It On: All or Nothing. After stepping away from acting for several years, she returned to the screen in 2023 with Scream VI.
Earlier this year, Panettiere published her memoir, This Is Me: A Reckoning, in which she openly discussed addiction, recovery and the difficult experiences she had faced throughout her life. She had also spoken about wanting to share her story in the hope that others would feel less alone.
The revelation of the official cause of death has now answered one of the biggest questions surrounding the actress’s sudden passing. However, authorities have made clear that the broader investigation has not yet been closed, leaving some details of what happened in the final hours of Hayden Panettiere’s life still under examination.
